1. Applications must be received on this form and address all questions and selection criteria.
2. The QV Care Awards is administered, funded and awarded by Ego Pharmaceuticals Pty Ltd (ABN 86 005 142 361) of 21-31 Malcolm Rd, Braeside VIC 3195 (‘Ego Pharmaceuticals’). Employees (and their immediate families) of Ego Pharmaceuticals are ineligible to enter. Immediate family means any of the following: spouse, ex-spouse, de-facto spouse, child or step-child (whether natural or by adoption), parent, step-parent, grandparent, step-grandparent, uncle, aunt, niece, nephew, brother, sister, step-brother, step-sister or 1st cousin.
3. Applications will be accepted from 25th of May 2025 to midnight 30th of June 2025 AEST. Applications must be submitted via the QV Care Awards website at https://www.qvskincare.com.au/qvcareawards2026.html. The awards are open to any Australian resident.
4. An application form may be completed by an applicant nominating herself or himself, or by nominating someone that you believe would be a worthy recipient of a QV Care Award. Nominees or applicants must complete the application form which sets out a criteria for each category (Community, Cause and Individual) of the QV Care Awards and demonstrate how the nominee or applicant meets that criteria. A nominee or applicant must provide the information required for the application to be deemed eligible. Application forms must be factual, non-defamatory, genuine, apolitical, must meet generally accepted community standards, and must not damage the reputation of Ego Pharmaceuticals.
5. Ego Pharmaceuticals reserves the right to withdraw and/or not award a category in any year.
6. Ego Pharmaceuticals will undertake assessment of all applications and their decision will be final and no correspondence will be entered into.
7. Late or incomplete applications will not be accepted.
8. Ego Pharmaceuticals will coordinate a judging panel who will assess all eligible applications. The judging panel will determine initially a short list of potential successful applications. Applications in the short list that contain a nominee, the person making the nomination shall be contacted to confirm consent from the nominee to proceed further in the process. If consent is obtained, the nominee shall be deemed an applicant from the point. If consent is not provided or obtained, the application will be withdrawn from the process.
9. Should an applicant be deemed successful, there will be a successful applicant for each category, who will each receive the sum of $10,000. Successful applicants are required to provide an Australian bank account details for payment of the award sum. Payment shall be made within 30 days of the announcement of successful applicants.
10. Successful applicants may be required to provide a brief biography and picture of themselves. Ego Pharmaceuticals should be acknowledged in any publication, social media post or presentation resulting from the award.
11. Ego Pharmaceuticals reserves the right to publish information about the QV Care Awards and successful applicants in their medium of choice. Ego Pharmaceuticals reserves the right to cancel or amend the award of QV Care Award where the successful applicant engages in conduct that is defamatory, political, does not meet generally understood community standards or threatens the reputation of Ego Pharmaceuticals.
12. Applicants should take their own advice in relation to tax implications of receiving a QV Care Award.
13. Any costs associated with applying, entering or participating in the QV Care Awards (including internet access and download charges) will be at the cost of the applicant.
14. Applicants agree to their entry being used by Ego Pharmaceuticals in the future and grant Ego Pharmaceuticals a non-exclusive, royalty-free, perpetual, worldwide, irrevocable and sub-licensable transferable licence to use, reproduce, modify, adapt, publish and display the entry for any purpose in any media without compensation, restriction on use, attribution or liability. Applicants warrant that they have full authority to grant these rights and will not assert any moral rights in respect of the entry. Ego Pharmaceuticals collects personal information (‘PI’) in order to conduct the scholarship program and may, for this purpose, disclose such PI to third parties, including but not limited to agents, and, as required, to Australian regulatory authorities. Entry is conditional on providing this PI. Ego Pharmaceuticals will also use and handle PI as set out in its Privacy Policy, which can be viewed here. In addition to any use that may be outlined in the Ego Pharmaceuticals’ Privacy Policy, Ego Pharmaceuticals may, for an indefinite period, unless otherwise advised, use the PI for promotional, marketing, publicity, research and profiling purposes, including sending electronic messages or telephoning the applicant. The Privacy Policy also contains information about how entrants may opt out, access, update or correct their PI, how entrants may complain about a breach of the Australian Privacy Principles or any other applicable law and how those complaints will be dealt with. All entries become the property of Ego Pharmaceuticals. Ego Pharmaceuticals will not disclose PI to any entity outside of Australia.
15. By submitting an application form, nominees and applicants confirm that all information submitted on the entry form is accurate and an applicant will submit substantiating information if required by Ego Pharmaceuticals. Applicants consent to receive marketing communications from QV and Ego Pharmaceuticals from time to time (unsubscribe at any time).
